Root cause analysis with 5 Whys and fishbone: causal statements and stronger actions

For supervisors, team leads, quality and operations staff, and anyone who runs or joins reviews of repeat problems or incidents. Complete 'Root cause analysis: 5 whys and fishbone' first. That course introduces the two tools. This one takes them further: writing the problem as a pattern, building a fishbone whose causes you can test, following more than one 5 Whys branch, writing causal statements that meet the Five Rules of Causation, and choosing stronger actions with an owner and a measure. The methods come from healthcare patient-safety guidance (AHRQ PSNet, IHI and the National Patient Safety Foundation's RCA²) and are applied here to a warehouse example.
